Adoption Application

Welcome! We’re so glad you’re here and considering a Life Unleashed Farm puppy. Our application is the first step in helping us get to know you, your lifestyle, and what you’re hoping for in a future furry family member.

We believe great matches don’t happen by accident — they’re built through thoughtful planning, honest communication, and a whole lot of love. This application helps us make sure each puppy is placed where they’ll truly thrive, setting both you and your future pup up for lifelong success.

Take your time, answer as thoroughly as you can, and don’t stress — there are no “perfect” answers here. We’re excited to learn more about you and guide you through the next steps of this journey!

If you have not already reviewed our adoption process and sale contract, please do so before preceding with the application. Both places will answer many, if not all, of the questions that you may have.

If you find you still have questions, please reach out to us or we can schedule a consultation once your application is approved.
